To approve the Rules of registration and issuance of a temporary certificate of a citizen of Ukraine (attached).

RULES
registration and issuance of a temporary certificate
citizen of Ukraine

1. Temporary identity card of a citizen of Ukraine (hereinafter - temporary identity card) - a document that identifies an individual and confirms his or her citizenship of Ukraine.

2. A temporary certificate shall be issued to persons who have reached the age of sixteen, acquired the citizenship of Ukraine and undertook to terminate foreign citizenship within two years from the date of acquisition of the citizenship of Ukraine. {Item 2 as amended in accordance with the Resolution of the Cabinet of Ministers No. 160 (160-2007-n) of February 7, 2007}

3. Temporary certificate is issued for up to 2 years. {Item 3 as amended in accordance with the Resolution of the Cabinet of Ministers No. 160 (160-2007-n) of February 7, 2007}

4. Temporary certificates shall be issued and issued to persons by the relevant LCA unit in the case of their residence in Ukraine, diplomatic missions and consular posts of Ukraine in other states - in the case of their residence abroad.

5. The following shall be submitted for registration of a temporary certificate:

application form according to the form established by the Ministry of Internal Affairs and the Ministry of Foreign Affairs; Paragraph two of item 5 as amended in accordance with the Resolution of the Cabinet of Ministers No. 810 (810-2009-n) of July 29, 2009 - the amendments were suspended in accordance with Presidential Decree No. 675/2009 (675/2009) of August 27, 2009; the amendments were declared unconstitutional on the basis of the Decision of the Constitutional Court No. 6-rp / 2010 (v006p710-10) of February 17, 2010; with changes made in accordance with the Resolution of the Cabinet of Ministers No. 559 (559-2010-n) of July 7, 2010}

2 photos measuring 35 x 45 millimeters;

certificate of registration of a person as a citizen of Ukraine.

Persons permanently residing in Ukraine shall submit a copy of the certificate of the state tax service on assigning them an identification number.

Documents are accepted subject to the presentation of an existing passport document or a document that replaces it and identifies the applicant. If necessary, a certified translation of the text of the document into Ukrainian is submitted.

6. The official of the bodies specified in paragraph 4 of these Rules, checks the submitted documents, in case of compliance with the established requirements registers them in a special journal and certifies the application form.

If necessary, the applicant is invited to provide additional explanations or clarifications.

7. The temporary certificate shall be issued and issued within ten days after the submission of the necessary documents.

8. The following information shall be entered in the temporary certificate:

last name, first name, and if available - patronymic;

citizenship;

date of birth;

Place of birth;

sex;

personal number;

details of the certificate.

The temporary certificate shall be signed by the official who executed it.

A photograph is pasted in the temporary certificate and the personal signature of the person to whom the certificate is issued is put.

Information about the person's place of residence is entered on the reverse side of the temporary identity card. {Item 8 is supplemented by a paragraph in accordance with the Resolution of the Cabinet of Ministers No. 160 (160-2007-n) of February 7, 2007}

9. All entries in the temporary certificate shall be made in the Ukrainian language.

10. It is prohibited to make entries in the temporary certificate that are not provided for in these Rules.

11. The accounting of temporary certificates is kept in a special journal, the form of which is approved by the Ministry of Internal Affairs and the Ministry of Foreign Affairs.

12. Temporary certificates are issued against a receipt.

13. The temporary certificate is the property of Ukraine.

14. Forms of temporary certificate are made by the printing plant "Ukraine".

15. A person must keep a temporary identity card securely. In case of damage or loss of the document, the person must immediately notify the authorized body. Re-issuance of a temporary certificate instead of a lost or damaged one is carried out in the manner prescribed by these Rules.

16. An interim certificate shall be issued to the relevant LCA unit or diplomatic mission or consular post of Ukraine in another state together with a document confirming termination of foreign citizenship or a declaration of renunciation of foreign citizenship, and the applicant shall be issued a Ukrainian passport.

17. Destruction of temporary certificates is carried out in accordance with the procedure established by the Ministry of Internal Affairs in coordination with the Ministry of Foreign Affairs.

18. Actions and inaction of officials who violate the procedure and deadlines for consideration of applications for the issuance of temporary certificates may be appealed in the prescribed manner to the authorized body of the highest level or to the court.
