In accordance with Article 10 of the Law of Ukraine “On Freedom of Movement and Free Choice of Residence in Ukraine”, the Cabinet of Ministers of Ukraine resolves:

  1. To approve the following:

Rules of registration of residence;

The procedure for the transfer of information by the registration authorities to the Unified State Demographic Register.

  1. To establish that:

electronic transfer of information on registration / deregistration of residence / stay of individuals from the registers of territorial communities to the Unified State Demographic Register is carried out after the commissioning of relevant hardware and software;

Terms

  1. These Rules determine the mechanism of registration / deregistration of residence / stay of persons in Ukraine, as well as establish the forms of documents required for this.
  2. In these Rules, the terms are used in the meaning given in the Law of Ukraine “On Freedom of Movement and Free Choice of Residence in Ukraine”.
  3. Registration / deregistration of residence / stay is carried out by the executive body of the village, settlement or city council, village head (if in accordance with the law the executive body of the village council is not formed) (hereinafter - the registration body) in the relevant administrative-territorial unit , which is subject to the powers of the relevant village, town or city council.
  4. A citizen of Ukraine, as well as a foreigner or a stateless person permanently or temporarily residing in Ukraine, are obliged to register their place of residence within 30 calendar days after deregistration and arrival at a new place of residence.

Registration of residence / stay or deregistration of a person's place of residence is carried out on the day of submission of documents by the person or his / her representative. Registration of residence at the request of the person may be carried out simultaneously with the removal from the previous place of residence.

  1. Parents or other legal representatives are obliged to register the place of residence of a newborn child within three months from the date of state registration of its birth.
  2. If a person cannot apply to the registration body on his / her own, registration may be carried out at the request of his / her legal representative or representative on the basis of a power of attorney certified in accordance with the procedure established by law (hereinafter - the representative).
  3. Registration of residence is carried out only at one address. If a person lives in two or more places, he registers the place of residence at one of these addresses of his choice. Official correspondence and official correspondence are served with the person at the address of the registered place of residence.
  4. Documents for registration / deregistration of residence / stay of a person are submitted to the registration authority (including through the center for administrative services), taking into account the requirements of the Law of Ukraine "On Administrative Services".

A person may instruct in the manner prescribed by law to submit an application for registration / deregistration of residence to the relevant official of the service management (maintenance) of an apartment building (dormitory), association of co-owners of an apartment building, housing cooperative, apartment building manager her residence.

At the request of the parents or one of them, the documents for registration of the place of residence of a newborn child may be submitted through the bodies of state registration of civil status during the state registration of the birth of a child.

Registration of the place of residence of a newborn child may also be carried out through the social protection authorities on the basis of data specified by the legal representative with whom the child resides, in the application for childbirth assistance.

Submission of an application for registration / deregistration of the place of residence of the homeless and other persons who do not have a permanent place of residence, deposit

applies to the relevant specialized social institution, institution of social services and social protection, where these persons live.

  1. Information on registration / deregistration of residence shall be entered in the passport of a citizen of Ukraine, temporary identity card of a citizen of Ukraine, permanent residence certificate, temporary residence certificate, refugee certificate, identity card of a person in need of additional protection, identity card of a person granted temporary protection (hereinafter - documents containing information about the place of residence), and information on registration of residence - to the certificate of application for protection in Ukraine (document containing information about the place of residence) by affixing the appropriate stamp of registration of residence / stay of a person in the form in accordance with Annex 1.

{Paragraph one of item 9 as amended in accordance with the Resolutions of the Cabinet of Ministers № 964 of 14.12.2016, № 579 of 09.08.2017}

Information on registration / deregistration of residence is entered in the passport of a citizen of Ukraine:

in the form of a booklet (1993 model) - by affixing a stamp of registration of the person's place of residence in the form in accordance with Annex 1 or a stamp of deregistration of the person's place of residence in the form in accordance with Annex 2;

in the form of a card (sample of 2015) - by entering information into the contactless electronic media, which is implanted in such a passport, in the case of workstations and connecting the registration authority to the Unified State Demographic Register (hereinafter - the Register). In case of non-connection of the registration body to the Register, the person is issued a certificate of registration or deregistration of residence, and entering information into the contactless electronic media is carried out by the territorial division of the LCA on the basis of such certificate.

  1. In case of entering erroneous information about the person during registration / deregistration of residence / stay, the registration authority is obliged to make the necessary changes to the documents containing information about the place of residence on the basis of reliable information provided by the person or his representative on the day of application. / stay, the register of the relevant territorial community and send information to the Register in accordance with the Procedure for the transfer of information by the registration authorities to the Unified State Demographic Register (hereinafter - the Procedure).
  2. The registration authority shall refuse registration / deregistration of residence if:

the person has not submitted the necessary documents or information;

the submitted documents contain inaccurate information or the submitted documents are invalid;

the person who has not reached 14 years has addressed.

The decision to refuse registration / deregistration of the place of residence is made on the day of the application of the person or his representative by indicating in the application for registration / deregistration of the place of residence the grounds for refusal. This application is returned to the person or his representative.

  1. Persons who do not live at the registered place of residence for more than one month and who have unfulfilled property obligations imposed administratively or by court decision, or are called up for military service and do not have deferment, or participate in court proceedings in of any quality, must notify the relevant registration authority in writing of their whereabouts.
  2. Provision of information on the person's place of residence and other personal data by registration bodies and LCA is carried out only in cases provided by the laws of Ukraine and only in the interests of national security, economic welfare and human rights or with the consent of the person upon request. .
  3. The period of storage of applications for registration of residence / stay and deregistration of residence of persons and, if necessary, relevant copies of documents submitted for registration of residence / stay or deregistration of residence in paper form is three years.
  4. Prior to the establishment of the register of the territorial community, the files on registration of persons transferred to the relevant registration authority, LCA, housing and utilities organizations and other enterprises, institutions and organizations that maintained files on registration of individuals shall be kept up to date. person and address card of the person in the appropriate forms in accordance with Annexes 4 and 5.
  5. In case of decision-making on change of numbering of houses, renaming of streets (avenues, boulevards, squares, lanes, quarters, etc.), settlements, administrative-territorial units, changes in administrative-territorial structure on the basis of the corresponding acts changes in the register of territorial community with the preservation of preliminary data and the subsequent entry of this information in the Register.

At the request of the person, this information is included free of charge in the documents, which indicate information about the place of residence / stay.

  1. In case of connection of registration bodies to the Register of entering information

information on the registration / deregistration of the place of residence / stay of a person to him is carried out on the application form, a sample of which is approved by order of the Ministry of Internal Affairs.

Residence registration

  1. To register a place of residence, a person or his representative shall submit:

1) application in the forms given in Annexes 6, 7 or 8, respectively;

2) a document in which information about the place of residence is entered. If the child has not reached 16 years of age, a birth certificate is submitted. Registration of the place of residence of children who are foreigners or stateless persons is subject to the entry of data on children in the certificate of permanent or temporary residence of their parents and a copy of the birth certificate. Documents issued by the competent authorities of foreign states are subject to legalization in the prescribed manner, unless otherwise provided by international treaties;

3) a receipt of payment of the administrative fee (in the case of registration of residence at the same time as removal from the previous place of residence, the administrative fee is charged for only one service);

4) documents confirming:

the right to live in a dwelling - a warrant, a certificate of ownership, a lease agreement (sublease, lease), a court decision that has entered into force, granting a person the right to move into a dwelling, recognizing a person's right to use housing or property rights on it, the right to register residence or other documents. In the absence of these documents, the person's place of residence is registered with the consent of the owner / co-owners, tenant and family members (these documents or consent are not required when registering the residence of minors at the address of residence of parents / parents or legal representatives / representatives);

the right to stay or be registered in a specialized social institution, institution of social service and social protection of a person - certificate of admission to service in a specialized social institution, institution of social service and social protection of a person in the form according to Annex 9, copy of certificate of registration registration of a homeless person, the form of which is approved by the Ministry of Social Policy (for persons registered in these institutions or establishments);

service in the military unit, the address of which is indicated during registration - a certificate of service in the military unit, issued by the commander of the military unit in the form in accordance with Annex 10 (for servicemen, except conscripts);

5) military card or certificate of registration (for citizens who are subject to military registration or are on the military register);

6) application for deregistration of the person's place of residence in the form in accordance with Annex 11 (in case of registration of the place of residence simultaneously with deregistration of the previous place of residence).

In case of submission of the application by the representative of the person, in addition to the specified documents, in addition are submitted:

identity document of the representative;

a document confirming the authority of the person as a representative, except in cases where the application is submitted by the legal representatives of the minor child - parents (adoptive parents).

Registration of a person's place of residence at the request of the legal representative is carried out with the consent of other legal representatives.

In case of registration of the place of residence of parents at different addresses, the place of residence of a child under 14 years of age shall be registered together with one of the parents with the written consent of the other parent in the presence of the applicant or on the basis of parents (except in cases where the child's place of residence is determined by a relevant court decision or a decision of the guardianship authority).

It is prohibited to require other documents to register a person's place of residence.

  1. An employee of the registration body or administrative service center shall check the validity of the document containing the place of residence (or the document containing the place of residence - in case of registration of the place of residence), the person who submitted it, its validity, the correctness of filling in the application for registration of residence / stay (if necessary, assists the person in filling out the application form) and the availability of documents required for registration of residence / stay, which he makes a corresponding entry in this application.
  2. In case of submission of documents required for registration of the place of residence of a newborn child through the bodies of state registration of civil status, the authorized employee verifies the identity of the parents (adoptive parents) or other legal representatives of the child and the document confirming the person's legal authority. representative, the person who provided it, their validity, the correctness of the application for registration of residence, the availability of documents required for registration of residence of the child, which he makes a corresponding entry in the

her statement. Accepted documents are submitted to the relevant registration authority at least once a month.

  1. The data necessary for the registration of the place of residence of a newborn child may be provided by one of the child's parents or his / her legal representative when applying to the social protection body for childbirth assistance. An employee of the social protection body shall check the documents submitted by one of the child's parents or his / her legal representative, taking into account the requirements provided for in paragraph 18 of these Rules. On the basis of the submitted documents, the employee of the social protection body forms the data according to the form in accordance with Annex 12 and transmits or sends them to the relevant registration body.
  2. In case of filing an application for registration of a homeless person's residence by a representative of a specialized social institution, social service and social protection institution or an official of apartment building (dormitory) management (maintenance) services, an association of apartment building co-owners, a housing cooperative , the manager of an apartment building, the documents specified in paragraph 18 of these Rules, at least once a week they submit to the registration authority for registration.
  3. Employee of the registration body on the day of application of the person or his representative or on the day of receipt of documents from the center of administrative services or a representative of a specialized social institution, social service and social protection, official of service management (maintenance) of an apartment building association of co-owners of an apartment building, housing cooperative, apartment building manager, bodies of state registration of civil status acts or data from the body of social protection of the population:

decides on the registration or refusal to register the place of residence of the person;

enters information on the registration of the place of residence in the document in which the information on the place of residence is entered (the document in which the information on the place of residence is entered - in case of registration of the place of residence). Date of registration of residence / stay of persons who submitted documents through the center of administrative services, a representative of a specialized social institution, institution of social services and social protection, an official of service providers for management (maintenance) of an apartment building (dormitory), association of apartment building co-owners , housing cooperative, apartment building manager, is the date of receipt of the relevant documents by the registration authority;

forms and enters data on registration of the place of residence of the person in the register of territorial community according to the Procedure;

generates information on the registration of the person's place of residence / stay for its transfer to the Register;

draws up and issues a certificate of registration of residence / stay for children under 16 and citizens who have a passport of a citizen of Ukraine in the form of a card, according to the forms in accordance with Annexes 13 or 14, respectively;

returns to the person or his representative, the administrator of the center of administrative services, a representative of a specialized social institution, institution of social services and social protection or an official of service providers for management (maintenance) of an apartment building (dormitory), association of apartment building co-owners, housing cooperative , the manager of the apartment building document, which includes information on the registration of residence, certificate of registration of residence, issued to the person in accordance with the sixth paragraph of this paragraph, and other documents submitted.

If the documents (data) for registration of the place of residence of the newborn child were submitted to the body of state registration of civil status or to the social protection bodies, the registration certificate issued by the registration body shall be returned to the relevant body of state registration of civil status. (representatives) who submitted the application may be issued by the relevant registration authority, administrative service center, sent by mail, as noted in the data on the newborn child, the form of which is given in Annex 12.

If during the submission of documents it is established that the person applied for registration of a new place of residence after 30 calendar days after deregistration of the previous place of residence or submitted an invalid passport of a citizen of Ukraine, the employee of the registration body or administrative service center administrative protocol for committing an offense specified in Article 197 of the Code of Ukraine on Administrative Offenses.

If a foreigner or a stateless person has submitted an invalid document for registration of residence, which includes information about

place of residence / stay, foreigner or stateless person is sent to the territorial subdivision of the LCA in accordance with the territory of service for taking administrative measures or other measures specified by the legislation of Ukraine.

  1. In case of registration of the place of residence simultaneously with deregistration of the previous place of residence in another administrative-territorial unit, a notification shall be sent to the registration body on the territory of which the person's previous place of residence was registered no later than the next day. in written or electronic form) on deregistration of the place of residence of a person in the form in accordance with Annex 15 for entering information into the relevant register of the territorial community.

In the identity document (in the form of a booklet), which contains information on registration of residence at the same time as deregistration of the previous place of residence, the registration authority first stamps deregistration of residence, and then the appropriate stamp of registration of new residence.

In the passport of a citizen of Ukraine (in the form of a card) information on registration / deregistration of residence is entered into a contactless electronic medium in accordance with the Procedure for registration, issuance, exchange, transfer, withdrawal, return to the state, destruction of the passport of a citizen of Ukraine. March 25, 2015 № 302 (Official Gazette of Ukraine, 2015, № 40, p. 1188).

If after the registration of the person's place of residence at the same time as deregistration of the previous place of residence it is established that the person did not report the registration of the place of residence with him at the previous address of minor children, both actions are canceled. .

Location registration

  1. Registration of the place of residence of persons who have applied for protection in Ukraine with an application form in accordance with Annex 8 shall be carried out for a period specified by law on the basis of documents specified in paragraph 18 of these Rules (except for receipt of administrative fee). Information on the registration of the place of residence is included in the certificate of application for protection in Ukraine, a sample of which is approved by order of the Ministry of Internal Affairs.

Deregistration of residence

  1. Deregistration of a person's place of residence is carried out on the basis of:

statements of the person or his representative in the form in accordance with Annex 11;

a court decision that has entered into force on deprivation of the right to own a dwelling or the right to use the dwelling, on eviction, on deregistration of a person's place of residence, on declaring a person missing or declaring him dead;

death certificates;

notification of the territorial body or subdivision of the State Migration Service indicating the relevant details of the passport of the deceased or the death certificate issued by the competent authority of the foreign state, legalized in the prescribed manner;

other documents certifying termination:

grounds for stay of foreigners and stateless persons on the territory of Ukraine (information of the territorial body of the LCA or territorial subdivision of the LCA, on the territory of which the person's place of residence is registered immigration and permanent residence permits in Ukraine);

grounds for living of a homeless person in a specialized social institution, social service and social protection institution (written notification of a social institution, social service institution and social protection);

grounds for the right to use the premises (expiration of the lease, rent, lease of premises, the period of study at the educational institution (in case of registration of residence in the dormitory of the educational institution for the period of study), alienation of housing and other documents specified by law).

Deregistration of residence on the grounds specified in the eighth and ninth paragraphs of this paragraph shall be carried out at the request of the authorized person of the specialized social institution, social service and social protection or at the request of the owner / tenant or their representatives.

Deregistration of the place of residence of orphans and children deprived of parental care, persons in respect of whom guardianship and custody has been established, is carried out in coordination with the guardianship and custody authorities.

Together with the application the person submits:

a document containing information on deregistration of residence. If the child has not reached 16 years of age, a birth certificate is submitted;

receipt of payment of the administrative fee;

military card or certificate of registration (for citizens who are subject to military registration or are on the military register).

If

submission of the application by the representative of the person, in addition to the above documents, are additionally submitted:

identity document of the representative;

a document confirming the authority of the person as a representative, except in cases where the application is submitted by the legal representatives of the minor child - parents (adoptive parents).

Deregistration of a person's place of residence at the request of the legal representative is carried out with the consent of other legal representatives.

In case of registration of the place of residence of parents at different addresses, deregistration of the place of residence of a child under 14 years of age together with one of the parents is carried out with the written consent of the other parent in the presence of the person accepting the application. consent of the other parent (except in cases where the child's place of residence is determined by a relevant court decision or a decision of the guardianship authority).

  1. An employee of the registration body or administrative services center checks the validity of the document containing information about the place of residence, the person who submitted it, its validity, the correctness of the application for deregistration of residence and the availability of documents required for deregistration residence, of which the said employee shall make a corresponding entry in this application.

An employee of the registration body on the day of the person's (representative's) request or on the day of receiving documents from the administrative services center, a representative of a specialized social institution, a social service and social protection institution, an official of apartment building (dormitory) management services. of co-owners of an apartment building, housing cooperative, manager of an apartment building:

decides on deregistration or refusal to deregister the place of residence of a person;

enters information on deregistration of residence in the document in which information on residence is entered. Date of deregistration of residence of persons who submitted documents through the center of administrative services, a representative of a specialized social institution, institution of social services and social protection, an official of service providers for management (maintenance) of an apartment building (dormitory), association of apartment building co-owners , housing cooperative, apartment building manager, is the date of receipt of the relevant documents by the registration authority;

forms and enters data on deregistration of a person's place of residence in the register of a territorial community;

generates information on deregistration of a person's place of residence for its transfer to the Register in accordance with the Procedure;

draws up and issues a certificate of deregistration of residence, the form of which is given in Annex 16, children under 16 years of age and citizens who have a passport of a citizen of Ukraine in the form of a card, and withdraws previously issued certificate of registration of residence for further destruction;

returns to the person or his representative, the administrator of the center of administrative services, a representative of a specialized social institution, institution of social services and social protection or an official of service providers for management (maintenance) of an apartment building (dormitory), association of co-owners of an apartment building, housing cooperative , the manager of an apartment building, a document which includes information on deregistration of residence, a certificate of deregistration of residence, as well as the documents submitted.

If during the submission of documents it is established that a person has submitted an invalid passport of a citizen of Ukraine for deregistration of residence, an employee of the registration authority or administrative service center shall draw up an administrative report for committing an offense under Article 197 of the Code of Administrative Offenses.

If a foreigner or a stateless person has submitted an invalid document for deregistration of residence, which includes information about the place of residence, the foreigner or stateless person is sent to the territorial division of the LCA in accordance with the service area to take administrative action. new document or making changes in the existing document specified by law.

If deregistration is carried out on the basis of the documents specified in paragraphs three, seven - ninth of paragraph 26 of these Rules, without providing a document containing information about the registration of residence, the stamp of deregistration is affixed in such documents in case of a person's application.

Deregistration of the place of residence in case of death is carried out on the date of receipt by the registration authority of the documents specified in paragraphs three to five of paragraph 26 of these Rules.

Unregistration / deregistration

  1. Registration of a place of residence / stay of a person or deregistration of a place of residence shall be revoked in case of their violation of the law.

In case of such violation, the head of the registration body checks the grounds for registration / deregistration of residence / stay of the person, based on its results draws a conclusion and decides to cancel the registration / deregistration of residence / stay of the person.

A written notification on the decision made shall be sent to the person in the form in accordance with Annex 17, which shall indicate the grounds for its adoption. The person is invited to an appointment with the registration authority to enter the relevant information in the document, which includes information about the place of residence / stay.

Employee of the registration body:

forms and enters data on cancellation of registration / deregistration of the place of residence / stay of a person in the register of the territorial community;

generates information on cancellation of registration / deregistration of residence / stay of a person for its transfer to the Register.

  1. In case of detection of violations of the law by the LCA or its territorial bodies, the registration body during the registration / deregistration of residence / stay of a person shall send a notification to such body regarding the need to conduct an appropriate inspection. In case of confirmation of the mentioned violations, the registration / deregistration of the place of residence / stay of the person shall be subject to cancellation.

Register of territorial community

  1. The registration body for the purposes of registration of residence / stay forms and maintains the register of the territorial community. The registration body is the administrator of the relevant register of the territorial community.

Registration authorities exchange information between the registers of different territorial communities for registration and deregistration of a person's place of residence in case of his / her departure from one administrative-territorial unit and arrival to another administrative-territorial unit.

Requirements for the collection, storage, processing, use and protection of information in the registers of territorial communities are determined by law.

  1. The following personal data shall be entered into the register of a territorial community:

last name, first name (all first names), patronymic;

date and place of birth;

place of residence / stay;

citizenship information;

unique number of the entry in the Register (if such information is entered in the passport of a citizen of Ukraine);

date of registration of residence;

last name, first name and patronymic of the representative, if he acts on behalf of the person, and information about the document certifying the authority of the representative;

information about previous place of residence;

date of deregistration of the place of residence (after the person's departure).

The name of the registration body, last name, first name, patronymic and position of the person who made entries in the register are also entered in the register of the territorial community.

  1. Access to information contained in the registers of territorial communities is carried out in compliance with the Laws of Ukraine "On Freedom of Movement and Free Choice of Residence in Ukraine", "On Information", "On Personal Data Protection" and "On the State Register of Voters" .

Officials and officials of registry administrators and other public authorities processing personal data entered in the registers are prohibited from disclosing personal data that became known to them in connection with the performance of official duties.

Every person has the right to access his / her personal data entered in the registers, as well as other rights regarding his / her personal data provided by the Law of Ukraine “On Personal Data Protection”.

Access to the registers is carried out exclusively authorized, with the preservation of information about the official or official who made the request, and the time of such a request.

The registration body is prohibited from transferring data from the register of the territorial community to third parties for purposes not provided for by the Law of Ukraine “On Freedom of Movement and Free Choice of Residence in Ukraine”.

The exchange of information between the registration authorities and other public authorities, local governments is carried out with the written consent of the person in order to provide him with administrative services.

The transfer of personal data entered in the registers of territorial communities to public authorities, local governments without the written consent of the person may be carried out only in cases provided by law.

In case of detection of erroneous information about a person in the register of territorial communities, the registration body shall notify the person within thirty days from the date of discovery of such information, and shall request the person to provide reliable information to amend the register and / or documents. information on the place of residence / stay, and transmits the relevant information to the Register.

Impersonal data from the registers of territorial communities are used for statistical purposes. Such data shall be submitted by the registration authorities to the respective bodies by the 5th day of the month following the reporting month

their territorial bodies of the State Statistics Service. Data from the registers of territorial communities can also be used for scientific purposes, provided they are depersonalized.

Control over the observance by the registration bodies of the legislation on the registration of the place of residence

  1. The LCA and its territorial bodies monitor the compliance of the registration authorities with the requirements of the legislation on registration of residence, as well as their activities in the field of registration / deregistration of residence / stay of persons.

Based on the results of the inspections, the LCA and its territorial bodies send to the registration authority an act on taking into account the recommendations (proposals) or eliminating the identified shortcomings and violations.

ORDER
transfer of information by registration authorities to the Unified State Demographic Register

  1. This Procedure determines the mechanism, forms and terms of transfer of information on registration / deregistration of residence / stay from executive bodies of village, settlement or city council, their officials, village heads (if according to the law the executive body of village council does not formed) (hereinafter - the registration authority), registering / deregistering the place of residence / stay (hereinafter - registration) of individuals in the relevant administrative-territorial unit, which is subject to the powers of the relevant village, town or city council, to the territorial authorities and LCA units to enter it into the departmental information system of the LCA of the Unified State Demographic Register.
  2. Information to the departmental information system of the LCA is transmitted on the day of registration:

by means of departmental information system of the register of territorial community in case of its connection to the Unified state demographic register (further - the Register) on the basis of the application form of the person for entering information in the Register which sample is approved by the order of the Ministry of Internal Affairs;

LCA software tools for entering registration information, posted on the official LCA website, or software interface tools for interaction with other software tools for maintaining registers of territorial communities.

The creation, administration and maintenance of registers of territorial communities and the mechanism for the transfer of information from the registers of territorial communities to the Register shall be carried out in accordance with the rules set out in Annex 1.

  1. Before connecting the departmental information system of the territorial community register to the Register or authorization of the registration body by LCA software, the registration information shall be transmitted within seven calendar days to the LCA territorial division according to the registration body's territory in paper and electronic form (Appendices 2 - 4). ).

The territorial subdivision of the LCA transmits the information received from the registration body to the relevant territorial body of the LCA for updating the data of its card index and entering information into the departmental information system of the LCA.

  1. In order to fulfill the tasks assigned to the LCA, to ensure the maintenance of the Register, territorial bodies and subdivisions of the LCA shall send requests to the registers.

territorial communities to check and clarify information about the place of residence / stay of persons. The term for consideration of these requests and submission of the answer to inquiries should not exceed three working days from the date of their receipt.

  1. Until the completion of the formation of the departmental information system of the LCA, the electronic file of the territorial body of the LCA shall be maintained in accordance with the procedure established by the LCA.

RULES
creation, administration, maintenance of the register of the territorial community (in terms of registration / deregistration of residence / stay of individuals) and the mechanism of transfer of relevant information by the registration authorities to the Unified State Demographic Register

  1. To create a register of a territorial community (hereinafter - the register) it is necessary:

1) LCA on its official website to create a page on which the executive body of the village, town or city council, its official, village head (if in accordance with the law the executive body of the village council is not formed), registering / deregistering the place residence / stay on the territory of the relevant administrative-territorial unit, which is subject to the powers of the relevant village, town or city council (hereinafter - the registration authority), will be able to apply for a register of territorial community (registration / deregistration of residence / stay persons);

2) develop technical requirements for the workstation with a set of equipment to ensure registration / deregistration of residence / stay of individuals and the communication channel for connecting to the Internet;

3) registration authority:

purchase and install a workstation and connect it to the Internet;

ensure that the employees of the registration body, which will be tasked with the registration / deregistration of residence / stay of individuals, electronic digital signature in one of the accredited key certification centers (electronic register of entities providing services related to electronic digital signature, and a list of accredited key certification centers are posted on the website of the central certification body of the Ministry of Justice: http://czo.gov.ua/ca-registry?type=4);

to form a mailbox in the domain * .gov.ua, or * .укр .;

apply for a register on the LCA website: at the link http://dmsu.gov.ua/ on the page "create a register", opening the window "apply for a register".

In this window you need to fill in the fields of the form placed in it:

full name of the registration authority (abbreviated name if available);

legal address of the registration authority;

details of registration in the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations;

last name, first name, patronymic of the head of the registration body, series and number of the passport, by whom and when issued, contact phone (work and mobile), e-mail address;

last name, first name, patronymic of the employee / employees of the registration body, who are tasked with registration / deregistration of the place of residence of citizens (in the absence of the employee / employees this field is not filled in).

The application must be signed with your own electronic digital signature after its formation and verification of the correctness of the entered data.

After the application is completed, the generated register will be available for use on the page "register of the territorial community".

  1. After creating the register, the employee of the registration body, which is responsible for registration / deregistration of residence / stay of individuals, must in the window "make an application for authorization" to draw up and sign his own electronic digital signature application for user login and access password to the register maintained by the relevant registration authority.

The application states:

full name of the registration authority (abbreviated name if available);

legal address of the registration authority;

details of registration in the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations;

last name, first name, patronymic of the employee of the registration body;

series and number of the passport, when and by whom it was issued;

contact phone (work and mobile);

email address.

After completing the application, the employee's e-mail login and password (111111) are sent to the employee's e-mail address to the register maintained by the relevant registration authority.

After receiving the notification, the employee of the registration authority must change the received password to his own.

Changing the password is done personally by the employee on the page "changing the access password". To create a password, you must use letters (upper and lower case) and numbers of the Latin keyboard layout (for example: Dj74YggE90).

After creating the password, you need to remember it. It is forbidden to transfer the password for access to the register to third parties.

  1. The register formed on the office

the site of the LCA, will ensure the implementation of the following functions:

storage of personal data entered in the register and their display in the prescribed form;

formation and printing of the application form for registration / deregistration of residence / stay of an individual;

formation and printing of the refusal to register / deregister the place of residence / stay of an individual;

verification of personal data of a person on the basis of databases formed with the use of personal data of the State Register of Voters;

formation and printing of the application form for entering information into the Unified State Demographic Register;

interrelated search by attributes, display of results in the interface, printing of a list or a card;

correction of data, re-registration of the place of residence / stay of individuals and cancellation of registration / deregistration of the place of residence / stay of individuals;

collection, preparation, input of information (data) from primary documents and scanned documents;

verification of the fact of registration / deregistration of the place of residence / stay of an individual at another address within the maintenance of the register;

search for information about an individual in the database of the register;

formation and printing according to the established forms of reports, certificates, cards, messages, etc.

  1. The application is formed by an authorized user (two-factor authorization) by filling in the fields of the relevant electronic application forms (windows of the graphical interface of the web service of the register) depending on the user-defined service: registration of residence, deregistration, deregistration simultaneously with the deregistration of the place of residence of the natural person at the previous place of residence.
  2. According to the data entered in the application for registration of residence / stay, data on a natural person shall be entered in the application form of the register:

last name, first name (all first names), patronymic;

date and place of birth;

place of residence;

citizenship information;

unique number of the entry in the Unified State Demographic Register (if such information is included in the passport of a citizen of Ukraine);

date of registration of residence;

last name, first name and patronymic of the representative, if he acts on behalf of an individual, and information about the document certifying the authority of the representative;

information about previous place of residence;

date of deregistration of the place of residence (after the departure of the individual).

The name of the registration body, last name, first name, patronymic and position of the person who made entries in the register are also entered in the application form of the register.

  1. To access the software of the registry, the employee of the registration authority must go to the Internet browser at: http://000.00.000.000/ and select the registry, which will appear in the appropriate window. The main form of user authorization in the registry should then appear.

The user who logs in to the registry enters his login in the "username" field, password in the "password" field and electronic digital signature, hovers the cursor over the "login" button and clicks it.

After passing the authorization, the user opens the register window of the relevant registration authority with the main menu, where he selects one of the items:

residence registration;

registration of location;

deregistration of residence;

cancellation of registration / deregistration of residence / stay of an individual.

  1. After the specified actions the corresponding form of input of the application form will open. The fields for entering the application form are marked with a red asterisk (*) in the fields required for data entry. The list of such fields includes:

type of registration;

grounds for registration;

city ​​(village) of place of registration;

house registration place.

If necessary, the user can enter data in the following form fields:

registration end date;

the need for the consent of the owner (co-owner) of the place of registration;

street of the place of registration;

building of the place of registration;

apartment of the place of registration.

When entering the address of residence, the user chooses the type of house possible place of registration from the list:

hostel;

House;

private house;

military base.

For the convenience of the user in the fields "city" and "street" built-in search and filtering mechanisms with additional buttons "address search filter" and "add a new street".

After entering the necessary data in the application form for registration / deregistration of residence / stay, the user clicks the "register" button to save the entered data and confirm the correctness of the entered data with his own electronic digital signature.

After saving the application form in the register, you can print it out by clicking the "Print application form" button.

If it is necessary to attach to the application form additional documents provided by the applicant / legal representative, or edit the entered data, the user can add them using the built-in input / editing mechanisms by clicking one of the "Add" or "Edit" buttons.

“Documents attached to the application by the applicant / legal representative” of the form of the identity card of the person of the register.

The user performs other functions of the register of the relevant territorial community by pressing the appropriate menu buttons and filling in the appropriate forms.

Completion of work in the register is carried out by pressing the appropriate button "Exit" and removing the media with an electronic digital signature.

  1. The transfer of information to the Unified State Demographic Register is carried out after the registration / deregistration of residence / stay of an individual by automatically forming the register of the relevant registration authority data sets entered in the application form of relevant individuals.

In the absence of complete data in the register of the relevant registration authority (absence of digitized facial image and / or digitized signature of the person) data sets for the state demographic register are not formed and sent, which is recorded in the register of the relevant registration authority. is not assigned to the state demographic register.

Upon receipt, information on registration / deregistration of residence / stay of an individual is stored in the departmental information system of the LCA and is used to draw up documents confirming the citizenship of Ukraine, identity or special status. After the identification of an individual, he / she is assigned a unique entry number in the Unified State Demographic Register.
